AB 104 & Grade Change Application

AB 104 & GRADE CHANGE APPLICATION
A component of AB 104 (new Education Code section 49066.5) is the ability of the
parents/guardians and adult student to request that high school grades earned during the 20-
21 school year be changed from letter grade to Pass or No Pass.
Parents of students shall have 15 days to complete the form below to request a grade change
for the 2020-21 school year. Fill in all fields, sign the form and submit it to your child’s school.
